Signature Healthcare at Home has been providing Payette and Malheur county with hospice and primary care services for over 25 years (previously as XL Hospice). From lowering unnecessary hospitalizations and emergency room visits to providing comfort and compassion during life’s most difficult time, our mission is to enhance the life of every person we serve.
Signature House Calls – Care Where You Are
Signature Primary Care involves a physician or nurse practitioner conducting on-site visits to patients who are too weak, no longer ambulatory, or too sick to leave their senior community. Often, primary care is also available to residents without a primary care doctor.
By providing these services to senior communities, residents and patients can receive the care necessary to stabilize their health. In addition to providing assessment and education, our doctors can prescribe medications, equipment orders, and more. Through our primary care services, we equip senior communities with the goal of lowering unnecessary hospitalizations and emergency room visits.
Access, availability, and a personal relationship with a nurse practitioner give patients freedom to take charge of all aspects of their personal health from getting a timely answer to a question about a new ache or pain or charting a new course for a healthy lifestyle to referrals to health care specialists and everything in between.

Our holistic approach to care during hospice includes a team dedicated to quality of life. The Signature team works as a group to create an individualized care plan based on the needs of the patient and family. This in turn is also shared with the family physician to keep everyone in constant communication on care and status.
